The ORSUS application enables the users to request service accounts for themselves as well as for their reportees. Access to these accounts in the target system will be granted after workflow (which is defined in the Identity Template) is completed.

| Template Name | Select the Template (that decides workflow, login name convention and other factors) based on which you would like to request service account. Use the Search button to select from the Identity Templates pop-up window and click Ok. |
| Identity Type & Identity Sub Type | You no need to enter this field. By default the system will take the data for these fields. |
| Owner Name | If you're requesting the account for yourself, you no need to select this field. If you're requesting for other user, then select the Owner name. Use the search button to select from the Owner Name pop-up window and click Ok. |
| First Name | Enter your first name or the first name of the user for whom you're requesting the account. |
| Last Name | Enter your last name or the last name of the user for whom you're requesting the account. |
| Display Name | Enter your display name or the display name of the user for whom you're requesting the account. |
| Login Name | Enter the login name for the service account you're requesting. |
| Justification | Provide the justification for the account you're requesting. |
After clicking Save, a Success Message displays indicating that Your Information Saved Successfully. Upon the request submission, you can check the workflow and provision status of the request submitted.
